Deficiencies observed: The person in charge did not identify the 5 symptoms and 5 illnesses for which employees need to be restricted and excluded. Several packages of string cheese were found being held and used in the facility past the manufacturer’s best by date. Employee phone and keys stored on top of equipment and single service items.

Deficiencies observed: Open package of sliced ham has sell by date of 1/10/2013 and unopened container of sour cream has use by date of 11/9/2012. Multiple food containers are covered with lids which have cracks/breaks. There is food debris and residue on the top of the knife holder.
