Violations: 12A, Violation of Code: [.04(4)(q) ] Bulk can of ketchup has been punctured in two places to allow dispensing. Open the container completely and transfer the unused remaining portion of the food into a food grade container. The unused portions of food from open cans must be transferred into a food grade container to prevent contamination via chemical changes in the can's lining. Food must be stored in a clean, dry location.  New Violation. 13A, Violation of Code: [.10(2)(g) ] (1.) The inspection report is on display at a bulletin board behind the counter where it is inaccessible to customers. Move the report to the wall behind the glass display case which holds the medicinal products for retail sale. The report must be posted within 15 feet of the entrance, 5 to 7 feet above the floor, and where a customer can read it while standing one foot away from it.  (2.) The old inspection report from June 2012 is posted. Must display the most recent inspection report at all times for customers to read.  Repeat Violation. 14C, Violation of Code: [.05(10)(g)(1)&(3) ] Unwrapped, disposable spoons and forks are stored without having all of the handles facing in a manner which allows the employee's hand to contact only the handle. Turn the tableware so that all of the handles are facing the employee. All tableware must be properly handled to protect the food-contact surfaces from contamination.  New Violation.
