April 15:

Name: Atlantic Buffet Sushi and Grill
Location: 2059 Scenic Highway, Suite 117
Score: N/A (This restaurant has not opened yet.)
Previous Scores: None
Deficiencies observed: No employee health policy present, no paper towels at hand sink, advisory regarding food borne illnesses not present, a notice to use clean tableware not present at self-service section not present for customers, and signs about washing hands are not present for employees. Because of these violations the restaurant was not given the OK to open, and a follow-up appointment was scheduled for April 25, 2011

Name: Carmike Cinema 12
Location: 1905 Scenic Highway, Suite 6000
Score: 92/A
Previous Scores: 100/A (November 29, 2010); 97/A (April 20, 2010)
Deficiencies observed: Hand sink at counter area with mold, employee serving popcorn not wearing hairnet, and exhaust hood filters above popcorn machine with oil build-up.

Name: T.J. Applebee's
Location: 1925 Highway 124
Score: 82/B
Previous Score: 92/A (November 16, 2010); 95/A (September 17, 2010)
Deficiencies observed: Can opener blade found stored with visible buildup of old food debris (repeat violation), cold foods at warmer temperatures, and an employee observed doing food preparation while wearing a watch. The restaurant was closed on April 15 due to its repeat violations. However, the restaurant was allowed to re-open that evening after correcting the problems, according to health officials reached April 19. There will be an updated inspection report made public.

April 14:

Name: Chick-fil-A
Location: 2932 Main Street
Score: 92/A
Previous Scores: 83/B (September 29, 2010); 96/A (August 11, 2010)
Deficiencies observed: Hand sink at prep area blocked with trash can, an in-use, wet wiping cloth station not at proper sanitizing solution level, and cutting boards with deep grooves that should be replaced.

April 13

Name: Sushi Avenue Japanese Restaurant
Location: 2118 Scenic Highway, Suite F
Score: 89/B
Previous Scores: 86/B (September 30, 2010); 88/B (April 27, 2010)
Deficiencies observed: An employee did not wash hands prior to using gloves, and rice dispensing utensils (not in use) were not stored with handles above the top of the rice storage container. The later was a repeat violation.

April 12

Name: Longhorn Steakhouse
Location: 1350 Scenic Highway
Score: 88/B
Previous Scores: 95/A (January 27, 2011); 79/C (January 21, 2011)
Deficiencies observed: Three cans in the dry storage area had dents and should not be used, and several containers of flavored syrups for desserts had expiration dates of June 2009
