Deficiencies observed: Employee observed changing gloves and continuing to work with food without washing hands before putting on clean gloves; ice machine had visible build up of mold-like substance; cold-held potentially hazardous foods not maintained below correct temperature; several single-service items, such as cups and straws stored found directly on the floor; hot water not available at the hand-washing sink in food prep area; and plumbing system needs work (several leaky sinks).

Deficiencies observed: Employee observed touching raw beef with gloved hands and then touching clean utensils and ready-to-eat food such as a bread roll with the same gloves, and raw beef stored above cooked meats and peppers.
