Deficiencies observed: An employee was observed placing cheese and pepperoni on pizzas with open wound on the side of left bare hand. Chicken wings holding since yesterday in the bottom portion of the prep cooler at 45F. Cold-held potentially hazardous foods not maintained below 41F. Wings were discarded.  Air temperature of the prep cooler is 42.6F. Air temperature of the holding unit is not at or below 41F.  Walk-in cooler curtains, walk-in cooler gasket, food dispensers, pizza cutter handles, and door have a buildup of dirtlike substance present; handles of the dough makers have buildup of food present; exterior of pans have a buildup of grease present. Nonfood-contact surfaces of equipment not kept free of an accumulation of dust, dirt, food residue, and other debris. This facility has 3 seats present. Person in charge to remove seating or provide a public restroom.
