Violations: 5-2, Violation of Code: [.04(7)(e) ] A proper consumer advisory is not in place for raw sushi items at self service bar. There is a disclosure and reminder statement however none of the raw sushi items are identified as being raw and linked to the statement, they are mixed with other fully cooked sushi items. The person in charge added proper labels to identify the raw items and link these with astiks to the disclosure and reminder statement.  Corrected On-Site.  New Violation. 6-1D, Violation of Code: [.04(6)(i) ] All items on time control including sushi rice, cut melon, cooked egg, bean sprouts, raw meat items among many others on the several hot and cold bars were not identified with discard times or color as outlined in the written procedure. The person in charge properly marked these items at the time of inspection. The person in charge was advised that all items on time control must be marked or otherwise identified with discard times as outlined in written procedures.  Corrected On-Site.  New Violation. 12A, Violation of Code: [.04(4)(l) ] Linen cloth rag in direct contact sitting on top of sushi rice. The person in charge removed the linen cloth. Linens and napkins should bot be used in direct contact with food. .  Corrected On-Site.  New Violation. 12A, Violation of Code: [.04(4)(u) ] Green onions are being displayed for self service without being protected from contamination. The person in charge removed this item. All items on display must be protected from contamination by being placed under proper sneeze guards or individually portioned etc.  Corrected On-Site.  Repeat Violation. 14C, Violation of Code: [.05(6)(r) ] The facility is reusing the soy sauce buckets to store food items such as other sauces, washed vegetables etc. The person in charge was advised these buckets are made for single use and are not easily cleanable for reuse, buckets should not be used to store food after first use.  Repeat Violation.
